We are seeking a skilled and compassionate Speech-Language Pathologist to join a Skilled Nursing Facility in Modesto, California. This role focuses on evaluating and treating patients with speech, language, communication, cognitive, and swallowing disorders within a geriatric and post-acute population. You will work closely with interdisciplinary healthcare teams to help patients improve communication abilities, cognitive function, and safe swallowing skills while enhancing overall quality of life. Job Responsibilities:
• Evaluate, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders in a Skilled Nursing Facility setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Provide therapy focused on speech production, cognitive communication, memory, language, and dysphagia management
• Collaborate with physicians, nurses, therapists, dietitians, and interdisciplinary teams to support patient recovery
• Educate patients and caregivers on communication strategies, swallowing precautions, and home exercise programs
• Monitor patient progress and modify treatment plans as needed
• Maintain accurate and timely documentation in compliance with facility and regulatory standards





Requirements

• Active state license as a Speech-Language Pathologist in California
• ASHA CCC-SLP certification preferred or required depending on facility policies
• Current BLS/CPR certification preferred
• Minimum of 1 year of recent SLP experience preferred, ideally in a SNF, rehabilitation, or geriatric setting
• Strong communication, clinical reasoning, and patient care skills
