Overview

Community Care Coordinator II (CNA OR LPN) - $6,000 Sign-On Bonus (to be paid throughout the first year of employment).

 

Beaver, PA - Full-time, Daylight shift, every other weekend

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

 

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

 

Community Care Coordinator II (CNA or LPN) provides Supervisor oversight of staff and facility.  Enhances the abundant life of participants through coordination of routine direct and indirect participant activities according to established policies and procedures.  Maintains participant rights, privacy, and confidentiality.  Additionally, the Community Care Coordinator II (CNA or LPN) promotes participant acceptance of care plans and responds promptly to overall participant needs.


Qualifications

Community Care Coordinator CNA or LPN requirements:

  • High school diploma or general education degree (GED); or 1 year related experience and/or training; or equivalent combination of education and experience.
  • CNA certification or LPN license
  • At least 1 year of experience with the frail or elderly population.

  • At least 6 months of experience as a Community Care Coordinator preferred.

  • Working knowledge of computers, Microsoft Word, and Excel required;
  • Excellent interpersonal skills, team-oriented, knowledge of medical terminology, computer literate.
  • Must have and present a current, valid driver's license and dependable transportation to visit clients' homes and other sites.
  • CPR and Certified First Aide required.
