Responsibilities include, but are not limited to: Assisting with personal care such as bathing, oral hygiene, nail care, foot care, skin care, and pressure relief Helping patients with feeding, dressing, transferring, positioning, and ambulation Accurately taking and recording vital signs (temperature, pulse, respiration, blood pressure) Notifying licensed staff when patients report pain or changes in condition Maintaining proper body alignment and safe transfer techniques Communicating observations clearly to the supervising RN Documenting patient care according to professional standards Collaborating with the Interdisciplinary Group Understanding and supporting hospice philosophy, including issues of death and dying Preserving the dignity, comfort, and respect of patients at all times.   About You: Must hold a CNA certification or have similar experience. Previous hospice experience preferred. Meets the training requirements of the State. Qualified candidates must have one (1) year direct patient care experience in acute or sub-acute care. Previous hospice experience is highly preferred. Superior customer service skills. Ability to read and follow written instructions and document care given. Travel is an essential function of this position and is required throughout the workday. Employees must possess and maintain a valid driver's license, reliable personal transportation prior to company vehicle assignment, and current automobile liability insurance that meets state minimum requirements. Employees are required to provide updated copies of their driver's license and proof of automobile insurance upon renewal or whenever requested by the Company. As a condition of continued employment in positions requiring driving, employees must successfully pass ongoing motor vehicle record (MVR) monitoring and maintain an acceptable driving record in accordance with Bridges Hospice policy and the Company's insurance requirements. Failure to maintain an acceptable driving record or eligibility to drive on behalf of the Company may affect an employee's ability to perform the essential functions of the position. While Bridges Hospice makes every effort to assign employees to a primary service area or geographic territory, business needs, patient care requirements, staffing needs, emergencies, and other operational circumstances may require employees to travel outside of their assigned territory. Employees are expected to be flexible and willing to provide coverage in other service areas as reasonably requested. The position requires standing, sitting, walking, bending, kneeling, stooping, crouching, reaching, and climbing throughout the majority of the day. The employee must frequently lift or move objects and patients weighing over 50 pounds. Routinely transfer patient weight 100lb or greater.
