We are seeking a skilled and adaptable Physical Therapist to join a combined Skilled Nursing Facility and Acute Care setting in Oak Valley. Job Responsibilities:
• Evaluate, diagnose, and treat patients in both SNF and acute care settings
• Develop and implement individualized treatment plans based on patient needs and medical status
• Collaborate with interdisciplinary teams to ensure coordinated patient care and safe discharge planning
• Supervise and work closely with Physical Therapist Assistants as needed
• Monitor patient progress and adjust interventions based on clinical response
• Educate patients and caregivers on mobility, safety, and recovery strategies
• Maintain accurate and timely documentation in compliance with facility and regulatory standards

Requirements
• Active state license as a Physical Therapist in California
• Current BLS/CPR certification
• Minimum of 1 year of recent PT experience preferred, with exposure to SNF or acute care settings
• Strong clinical decision-making and adaptability in fast-paced environments
• Ability to work collaboratively within a multidisciplinary team
