Overview

JOB SUMMARY:

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

Responsibilities

Promotes a service culture.

Demonstrates the customer satisfaction standards set for the institution.

Actively participates in ways to prevent customer complaints and dissatisfaction with services.

Focuses attention and actions on what is best for the patient or customer.

Attempts to identify all customer needs and tries to meet and/or exceed expectations.

Interacts with all customers in a caring manner.

Contact with others is polite and proactive.

Promotes a positive teamwork environment.

Creates closure after interactions.

Communicates effectively in a positive, respectful and concise manner.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

High School Diploma

Two years of acute care experience.

BCLS (AHA) certification within 60 days of hire and maintain.
