Overview

Monterey Park Hospital, a 101-acute care facility located in the San Gabriel Valley of Los Angeles County, is seeking a Social Worker for our Utilization Review Department. This is a full-time, 8-hour day shift position reporting to the Chief Operating Officer.

Responsibilities

The Social Worker provides direct social work services and counseling to patients and their families, provides psychosocial and diagnostic impressions on all patients seen, evaluates social/family information, and determines a plan for social treatment. Consults with physicians and health care personnel about patients or situations which directly impact patients' illness/hospitalization, maintains a continuity of care by following up with patients referred for Social Service Intervention, assists in the provision of crisis casework to Emergency Dept. patients referred by that department for assessment, counseling, and referral.  Coordinates a family conference in which the patient's goals and progress may be discussed.  Maintains close working relationship with all appropriate community agencies. Participates in hospital in-service programs regarding the social and emotional components of illness and their effects on patients/families.  Performs basic management tasks related to the job, such as time planning, recording, reports, statistics, and other routines as required by the Utilization Review Department.  Attends appropriate department and hospital in-services, participates in professional organizations and activities outside as they relate to the position, and assists in departmental studies and projects as assigned by the Director.

Qualifications

  • LCSW or MSW degree required.
  • 2-3 years of acute care experience in a hospital setting preferred.
