Clinical Coordinator for Palliative Care

VNA is looking for a Clinical Coordinator. 

Job Summary: The Clinical Coordinator is responsible for identifying and coordinating nursing care needs for patients and their families, while providing ongoing supportive care to patients, caregivers, and loved ones. In addition, the Clinical Coordinator directs and coordinates care for palliative patients, ensuring appropriate placement within VNA service lines as patient needs evolve. Using the nursing process, the Clinical Coordinator focuses on delivering comfort, comprehensive care, and maximizing quality of life for both patients and families.

Status: Full-Time 

Schedule: Monday - Friday

Education/Experience/Skills/Qualifications: 

• Current Registered Nurse licensed to practice in the State of California
• Current CPR certification
• 2 years of Home Health and/or Hospice experience 

Special Requirements: 

• Current driver's license
• Auto insurance
• Reliable vehicle

The compensation range for this role is determined by factors such as location and required skills. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range. Pay decisions are based on the unique circumstances of each candidate and position.
