ACCOUNTABLE FOR:  

The Retreat Registered Nurse, with the assistance of the Director of Nursing, is responsible for planning, counseling, implementing, and delivery of services by the nursing staff to ensure the satisfaction of the Residents and Family.  The RN is responsible to assure the planned care is delivered in a safe, economical and efficient manner, which follows facility’s policies and procedures

GENERAL PURPOSE AND ESSENTIAL FUNCTIONS: 

Responsibilities and Duties:

  1. Helps to oversee patient care to ensure the satisfaction of Residents and Family by reviewing resident status, developing relationships with residents and families and resolving problems and complaints.
  2. Responsible for Stand Up Meetings
    1. a. Notify C.N.A.s at the beginning of each shift of any pertinent information related to resident safety, quality of care, appointments, etc.
  1. Facility Rounds
    1. a. Make rounds of the building and have appropriate staff correct any findings.
    2. b. Rounds are to be completed throughout shift.
  1. Employee and Staffing
    1. a. Print and/or post Daily Assignment Sheet
    2. b. Print daily census
    3. c. Ensure adequate staff to meet facility needs
    4. d. Receive call outs and ensure staffing meets facility needs
    5. e. Contact RNH employees to cover shifts before notifying agency
    6. f. Work as Charge Nurse if there is a nurse call out for 7A-7P shift
    7. g. Address any employee concerns as appropriate
  1. Resident Care
    1. a. Hospital returns on weekends – Reconcile and enter medications in Matrix (E.H.R.) and assist charge nurse as needed.
    2. b. Monitor and assist in the Dining Room during breakfast, lunch, and supper meals.
    3. c. Complete incident report forms for any resident injuries of unknown, altercations, or other abuse issues according to facility policy.  Call D.O.N. &/or RNH Administrator
    4. d. At 2:30 PM, print POC to ensure meal intake and BMs are recorded, notify C.N.A. if not recorded
  1. Document all pertinent information in Matrix (E.H.R.)
  2. Treatments:
    1. a. Complete treatments as ordered and document in Matrix (E.H.R.)
    2. b. Order and document any new treatment needs
    3. c. Monitor and assist staff with the skin prevention program
    4. d. Monitor & assist staff with the bowel & bladder program
  1. Address any family or resident concerns or grievances as appropriate and document as indicated.
  2. Mail Notification of bed hold letter to responsible party when a resident is admitted to the hospital on Saturday or Sundays and put a copy in Rena’s box. (Medicaid residents only)
  3. Complete incident report for employee or visitor accidents.  
  4. Contact the Administrator on Call as needed for assistance.
  5. Assist with activities as needed.
  6. Keep portable phone to answer calls.
  7. Distribute mail to residents as appropriate.

 

 Related Functions:

  1. Arrives for the scheduled shift no earlier than fifteen (15) minutes before the shift begins and clocks out no later than fifteen (15) minutes after the scheduled shift ends.
  2. Ability to use initiative, judgment, and resourcefulness to make decisions.
  3. Demonstrates loyalty to the organization, honesty, and integrity at all times in the care and use of patient / resident and facility property.
  4. Demonstrates positive support of any and all programs / projects implemented by the Director of Nurses or Administration.
  5. Maintains confidentiality of patient / resident information.
  6. Displays a neat, clean, and professional appearance at all times.
  7. Cooperates when needed if the facility faces an emergency or unusual circumstance.
  8. May be asked to participate on committees and community projects.
  9. Promotes good public relations between nursing home staff, patients, families, visitors, and medical staff. 
  10. Exhibits an attitude that promotes harmony and goodwill in the workplace.
  11.  Promotes facility culture by providing quality, comprehensive services to all customers through a team approach.
  12. Use proper body mechanics for transfers, lifting, etc…
  13. Provides duties as the Charge Nurse and / or Treatment Nurse as needed.
  14. All other duties as assigned.
