Job Description:

  • JOB SUMMARY
    • Valued member of patient care team in ambulatory setting. Optimize and follow up on patient therapies as well as being an invaluable resource for the providers.
    • DUTIES AND ESSENTIAL FUNCTIONS
      • Dispenses medications upon receiving, reviewing, and verifying written orders from a provider.  Reviews and interprets new medication orders and patient profile to assure appropriate drug, dose, and route.  Reviews for therapeutic duplications, allergies and drug interactions.  Contacts and confers with providers.
      • Provides patient education for medication therapy. Proactively participates in patient care, reviewing patient profile, providing pharmacotherapeutic consults, making patient specific drug therapy recommendations.  Document clinical consults as part of department documentation program.  Actively participates in medication reconciliation process including medication history and discharge counseling. 
      • Actively participates in the development and implementation of performance/quality improvement and patient safety activities for clinic.  Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved.  Document cost avoidance as part of department documentation program. 
      • Enhances professional and technical knowledge and competency by attending class work/training sessions and journal clubs.  Attend professional meetings. 
      • Supervise pharmacy students in clinical duties within the practice.
      • Assists in the reproduction of services in other specialty clinics.
    • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
      • Minimum requirement:
        • Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy or PharmD degree
        • Full Time Status
        • State of Kentucky Pharmacy license
        • Functional knowledge of Joint Commission regulations
        • Functional knowledge of federal and state regulations
      • Preferred qualifications:
        • BCACP or other Pharmacy Certification Preferred
        • Member of a professional organization 
    • WORKING ENVIRONMENT
      • Constantly works near moving mechanical parts
      • Constantly exposed to blood and/or body fluids and infection. 
      • Occasionally exposed to toxic or caustic chemicals
      • Occasionally exposed to risk of radiation
      • The noise level in the work environment is usually moderate.
    • PHYSICAL DEMANDS
      • Occasionally required to maintain in a stationary position (standing or sitting).
      • Frequently required to walk and move about.
      • Frequently required to operate a computer and other equipment.
      • Frequently required to use fine motor skills to handle and operate instruments/devices.

      • Constantly required to communicate with patients and other care team requiring talking and hearing.

Physical Demands

This position requires intermittent sitting at a computer workstation for extended periods of time; regular standing, walking, and being on feet for long periods; performing tasks with repetitive motions such as typing and handling materials; and communicating effectively with staff, patients, and visitors. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 100 pounds, and occasionally lift and/or move more than 100 pounds. The role also requires the ability to tolerate personal protective equipment (PPE) for long durations and demands specific vision abilities including close vision for reading and computer work.
