Principal Accountabilities:


1. Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
2. Collaborates with the referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control. 
3. Maintains professional education standards required to be a CDCES in an ADA approved Diabetes Self- Management Education program.
4. Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
5. Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
6. Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
7. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
8. Communicates patient information to referring providers and appropriate staff in a timely manner.
9. Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.
10. Performs other related duties as assigned.

11.  Position is at two locations, with a schedule of two days per week at 435 South Street, Morristown  and two days per week at the Rockaway Pavilion. 

Qualifications

Requirements:

1. Bachelor's degree in nursing, food, nutrition, dietetics or related field.
2. License to practice as a Registered Professional Nurse.
3. Three years of experience as a Diabetes Educator or relevant.

4. Obtain CDCES within 1 year.



Preferred:
1. BSN preferred.
2. CDCES preferred.
3. Epic experience preferred.

4. Bilingual (English/Spanish) preferred.
