Job Overview

Paradise Creek Health and Rehabilitation of Cascadia is looking for dedicated and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team! Key Responsibilities

  • Administer medications and treatments as prescribed
  • Monitor residents' health status and report any changes to the healthcare team
  • Develop and implement individualized care plans in collaboration with residents, families, and the interdisciplinary team
  • Supervise and support Certified Nursing Assistants (CNAs) in delivering resident care
  • Assist with admission, discharge, and transfer processes
  • Ensure accurate and timely documentation in accordance with state and federal regulations
  • Provide emotional support to residents and their families
  • Participate in ongoing education and training to maintain clinical competence

Qualifications

  • Current LPN or RN license in the state
  • Strong clinical and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively as part of a team
  • Compassionate and patient-centered approach to care
  • Previous experience in long-term care, geriatric, or rehabilitation settings is a plus
  • Ability to manage multiple tasks and prioritize effectively

Education

Graduate of accredited LPN School of Nursing.


Licenses/Certification Ā 

Current valid LPN license in the state employed.

Valid CPR certification.


Experience
