Overview

As a Behavioral Health Clinician (BHC), you will serve as an integral member of the primary care team, working in a consultative and collaborative role to improve the overall health and well-being of patients. You will help address a broad spectrum of behavioral health needs by providing early identification, assessment, triage, and evidence-based, problem-focused interventions for patients experiencing behavioral, emotional, psychological, and social concerns related to both physical and mental health conditions.

 

In this role, you will meet with patients individually to conduct comprehensive assessments, develop recommendations that enhance the patient's care plan, and collaborate closely with primary care providers to ensure coordinated, patient-centered care. When appropriate, you will provide follow-up treatment through skills training, coaching, and evidence-based interventions designed to improve patients' health, functioning, and overall wellness. You will also play a key role in supporting patients who require services beyond those available within the clinic by facilitating smooth transitions to community-based behavioral health resources.

Responsibilities

  • This position requires advanced level skill in delivering evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems.
  • The work includes the BHC meeting with patients individually for an assessment and then providing recommendations to the primary care team to enhance the patient's care plan. 
  • When indicated, the BHC may also work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning.
  • The BHC also supports helping patients, who require services not provided in the LMG clinic, to make a smooth transition to care with community partners.

Qualifications

Education:

  • Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.

Experience:

  • Minimum two years of clinical treatment experience with patients and working as part of a multidisciplinary care team.

Skills:

  • Ability to diagnose and treat mental health and substance use disorders for the identified clinic population.
  • Competent to deliver evidenced based therapies such as cognitive behavioral therapy, acceptance and commitment therapy, short term psychodynamic psychotherapy, dialectal behavioral therapy, trauma focused therapy and solution focused interventions.
  • Ability to collaborate and advocate for patients’ behavioral and physical health needs with the referring provider.
  • Ability to engage patients in health behavior change through brief interventions and health/psychoeducation.
  • Coordinate with referral resources and other clinic providers and, where appropriate, community partners to facilitate referral to appropriate specialty treatment.
  • Highly adept social skills, with strong professional presence.
  • Demonstrated competencies in diagnostic assessment of emotional, cognitive, behavioral and substance use disorders for population(s) served in clinical settings. 
  • Demonstrated competency in assessment of cognitive and psychological problems associated with and/or exacerbating medical conditions. 
  • Experience with delivering trauma informed care. 
  • Experience with individual psycho/social treatment and conducting groups.
  • Experience treating patients with acute and chronic physical health conditions.

Licensure

  • Licensed in one of the following within applicable state:
    • Clinical Social Worker (LCSW)
    • Licensed Independent Clinical Social Worker (LICSW)
    • Licensed Professional Counselor (LPC)
    • Licensed Mental Health Counselor (LMHC)
    • Licensed Marriage and Family Therapist (LMFT)

Pay Range

USD $43.11 - USD $64.22 /Hr.
