We are seeking a compassionate, organized, and dedicated Case Manager to join our team. This position plays a critical role in helping individuals access resources, achieve recovery goals, and maintain long-term success.


Position Overview

The Case Manager works under the supervision of the Lead Case Manager to manage assigned caseloads, provide community and office-based support, and ensure compliance with agency policies and regulatory requirements. The ideal candidate is empathetic, detail-oriented, and skilled at balancing direct client support with administrative and documentation responsibilities.


Key Responsibilities

  • Maintain assigned caseloads and ensure regular client contact.
  • Monitor client progress and assess ongoing needs in accordance with agency policies, contracts, and best practices.
  • Meet with clients individually and in group settings.
  • Collaborate with families, clinicians, case managers, and other community partners.
  • Provide crisis intervention, advocacy, resource coordination, financial assistance support, and assistance with activities of daily living (ADLs).
  • Conduct community visits, client monitoring, and transportation as needed.
  • Develop service plans and complete clinical documentation using Electronic Health Record (EHR) systems.
  • Maintain accurate, timely, and compliant client records.
  • Conduct family outreach and participate in the Loved Ones Group.
  • Communicate client progress and updates with probation departments and other referral sources as appropriate.
  • Attend required trainings, staff meetings, and agency events.
  • Contribute to a safe, supportive, and recovery-focused environment.


Qualifications Required

  • Valid driver's license.
  • High school diploma or equivalent.
  • Active Chemical Dependency Counselor Assistant (CDCA) certification or ability to obtain within 30 days of hire.


Preferred

  • Bachelor's Degree in Human Services, Criminal Justice, Social Work, Counseling, or a related field.
