Admissions & Benefits Coordinator

We are seeking experienced and highly organized Admissions & Benefits Coordinator to support the admissions process while helping individuals and families navigate benefits, entitlements, and service eligibility.
This is an outstanding opportunity for a professional who enjoys problem-solving, working directly with families, coordinating documentation, and ensuring individuals receive the services and financial supports they need. We are looking for someone who is compassionate, detail-oriented, customer-service driven, and able to manage sensitive information with professionalism and urgency.
Key Responsibilities:
- Coordinate admissions, intake, and transition processes for new individuals entering services
- Assist families and stakeholders with benefits and entitlement processes, including Medicaid, Social Security, and related documentation
- Monitor eligibility requirements, track timelines, and maintain organized records to support continuity of services
- Serve as a key point of contact for families, care coordinators, hospitals, and referral sources throughout the admissions process
- Collaborate with interdisciplinary teams to ensure smooth transitions, accurate documentation, and timely service coordination
Qualifications:
- Experience in healthcare, human services, admissions, benefits coordination, case management, or a related field preferred
- Knowledge of Medicaid, Social Security benefits, entitlement systems, or OPWDD-related services strongly preferred
- Strong communication, organizational, and follow-through skills
- Ability to manage multiple priorities and maintain confidentiality
- Associate’s or Bachelor’s degree preferred depending on experience
