Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator II – School-Based to support youth and families within a school setting. This role focuses on coordinating care, providing support, and helping students succeed both emotionally and academically.
Key Responsibilities
- Coordinate behavioral health, medical, and social services
- Assist with treatment planning under a licensed clinician
- Provide individual and group social-emotional learning (SEL) sessions
- Support students and families in crisis situations
- Collaborate with school staff, families, and providers
- Monitor progress and maintain documentation in the EHR
Qualifications
- Bachelor’s or Master’s degree in a related field (Social Work, Counseling, Psychology, etc.)
- Valid driver’s license
- Strong communication and organizational skills
⭐ Preferred
- School-based or youth behavioral health experience
- Familiarity with CCBHC
- Licensure (LSW, RN, or similar)
