Hospice Liaison/Account Executive
Schedule: Monday - Friday 8:00am - 4:30pm

SUMMARYÂ
The Community Educator (CE) informs those in the health care field about the Company and the services we provide. The CE professionally represents the Company to attain and maintain accounts throughout the community. May attend discharge/multidisciplinary rounds in acute care, ambulatory and or other settings within the health system to share expertise and to assist the patient in transition of care from one setting to the next within the health system. The CE complies with company policies and follows all legal rules and regulations.Â

KEY RESPONSIBILITIESÂ
Educate those in the healthcare field regarding the Company services; i.e.: physician’s offices, long-term care facilities, rehabs and hospitalsÂ
Develop and maintain relationships with all healthcare professionals in the surrounding communitiesÂ
Create awareness in the community of the need and benefits of home healthcare servicesÂ
Network and attend business organizations on behalf of the CompanyÂ
Develop and organize educational programs for the community and healthcare workersÂ
Maintain existing accountsÂ
Educate clients and prepare them for servicesÂ
Coordinate new referral admission process to ease transition by working closely with the Company internal staffÂ
Follow-up with referrals to assess how service is going through phone calls, letters and visitsÂ
Prepare weekly marketing reports for supervisorÂ
Prepare monthly expense reportsÂ
Set weekly, monthly and yearly goals in obtaining new accounts, referrals and customersÂ
Attends in-service trainings and mandatory agency meetingsÂ
Maintains a professional appearance as a representative of the companyÂ
Attend on-going educational seminars to keep updated on new trends in Medicare, Medicaid, Home Healthcare policies, etc.Â

EDUCATION AND CREDENTIALSÂ
Bachelor’s degree in business, marketing, or related field or 4 years of healthcare sales experience, preferably in homecare or hospiceÂ
