We are seeking a skilled and passionate Occupational Therapist to join our team at Creative Health Solutions. The ideal candidate will work closely with a team of healthcare professionals to develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.

Responsibilities
As an Occupational Therapist you will assist in:聽聽路Review available patient information and evaluate the impact of this information on the assessment and treatment process.路Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures. Includes home/vocational and equipment needs.路Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals. 路Revise treatment plans, as needed, throughout the treatment process.路Completes documentation in accordance with applicable federal and state laws and applicable standards and administrative and clinical requirements.路Provides supervision to Assistants in accordance with applicable State Practice Act guidelines. 路Reports to work as scheduled.路Orient patients to program services and treatment procedures appropriate to their needs.聽 Inform patients of any potential risk during any procedure.路Communicates with nursing staff, patient and families to ensure effective care delivery.路Establish and instruct patient and family/other caregiver in exercises and therapeutic procedures to be continued following discharge. 路Assist in the referral process when a patient requires additional services following discharge.聽 Maintain current knowledge of community resources. 路Consistently records outcome scoring data accurately.路Consistently utilizes appropriate treatment interventions for each patient served that are individualized and ensure appropriate level of challenge, sophistication, and complexity to facilitate maximal patient progress and outcomes.路Attend and participate in rehabilitation team conferences regarding patient progress, problems or needs.路Attend and participate in other department/facility meetings, as required.路Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical accuracy. 路Act in compliance with CHS policies and procedures, State, Federal and Professional Organization regulatory and professional standards and guidelines. 路Provide input to the Rehab Director on clinical performance of assistants as requested and appropriate.路Perform other duties as assigned.聽IND123Qualifications
Minimum Qualifications/Requirements:

聽
路Master of Science degree in Occupational Therapy or Bachelor of Science degree in Occupational Therapy and five years of direct practice experience. 路Current license or eligible for license under statutes in the state of practice.路Demonstrates competency in applying the principles, methods, materials and equipment used in Occupational Therapy to the patient population served by this facility.路Excellent oral and written communication and interpersonal skills.路Adequate computer skills in order to complete required job duties and computerized documentation.路Demonstrate ability to assess patient needs and develop and implement a comprehensive plan of care.路Strong organizational skills in order to effectively maintain multiple projects simultaneously.路Requires creativity as well as the ability to work independently.Learn more about this Employer on their Career Site
