Service Coordinator – TennCare CHOICES
Location: Murfreesboro, TN Job Type: Full-Time Salary: $58,000–$62,000 annually

Charis Hearts is seeking an experienced, organized, and compassionate Service Coordinator to join our growing TennCare CHOICES Division in Murfreesboro.
This is far more than a scheduling position. As a Service Coordinator, you'll oversee daily operations for a caseload of members receiving in-home services, ensuring they receive reliable, high-quality care that allows them to remain safely and independently in their homes.
You'll work closely with caregivers, members, families, recruiters, and Managed Care Organizations (MCOs) to coordinate services, solve challenges, support compliance, and ensure exceptional service delivery.

What You'll Do
Member & Caregiver Coordination
· Coordinate daily caregiver schedules for assigned members.
· Ensure members receive uninterrupted, high-quality services.
· Resolve call-offs, staffing shortages, and scheduling conflicts.
· Communicate with members, caregivers, family representatives, and referral sources.
· Conduct client admissions and coordinate new starts of care.
· Build positive relationships while providing exceptional customer service.
Operations & Staffing
· Recruit, interview, hire, and onboard caregivers.
· Partner with recruiting to fill open cases quickly.
· Monitor staffing coverage and authorization utilization.
· Track hospitalizations, service holds, missed visits, and staffing trends.
· Payroll verification through accurate scheduling and visit review.
· Maintain accurate client and caregiver records within AxisCare.
Compliance & Quality
· Monitor Electronic Visit Verification (EVV) activity and scheduling accuracy.
· Review documentation and resolve visit exceptions.
· Ensure services are delivered according to authorized hours and care plans.
· Maintain documentation that supports state and MCO compliance requirements.
· Participate in quality improvement initiatives and operational audits.

We're Looking for Someone Who Has
· Two or more years of experience in one or more of the following:
o Service Coordinator
o Home Health Coordinator
o Client Care Coordinator and staff oversight
o Staffing or workforce scheduling for high volume region
o Medicaid waiver or long-term services and supports
· Excellent organizational and time management skills.
· Strong communication and relationship-building abilities.
· Confidence making decisions in a fast-paced environment.
· The ability to manage multiple priorities while maintaining exceptional attention to detail.
· Proficiency with Microsoft Office, Microsoft Teams, email, and computer systems.
