TITLE: 聽Community Growth Integration Coordinator聽

STATUS: 聽FT/Regular

CLASSIFICATION: 聽聽Exempt

PROGRAM HOURS: Exempt 聽

POSITIONS SUPERVISED: 聽Direct Support Professionals/Associate Coordinators

SALARY: 聽 $70,304 per year 聽

*On-Call Stipend per week on-call

DEFINITION: 聽Under the supervision of the Director assume responsibility for operations of organization and assure agreed upon delivery of support services.



INTERACTION/ORGANIZATIONAL/SOCIAL SKILLS:

路 Must be able to relate positively to individuals, their families/guardians, neighbors, Regional Center representatives and other community members

路 Must possess the belief and attitude that all people with developmental disabilities can be served in their communities.

路 Must be capable of using language that portrays people with developmental disabilities in a way other than the typical medical model.聽

路 Must possess the belief that a developmental disability is not an illness.

路 Must be emotionally capable of examining one鈥檚 self and one鈥檚 own issues with power and control when supporting and acting in the service of other people.

路 Must have the ability to communicate in the language spoken by the individual and in writing to the individuals served, supervisors and other team members

路 Must be able to effectively organize a variety of work tasks for self and complete these in priority order

路 Must have own transportation, valid California driver鈥檚 license and insurable driving record and satisfy the minimum State requirement regarding auto insurance.

路 Must be able to liaison w/ families- assure that family members who desire communication receive weekly or more frequent phone calls if the individual agrees or they are the conservator.聽

PHYSICAL REQUIREMENTS:聽

路 Duties require up to 50% of work time be spent traveling from one place to another.

路 Must have the ability to effectively implement First Aid/CPR/CPI procedures.

路 Must have the ability to work on multiple assignments at one time.

路 Must have the ability to push people using wheelchairs and assist people
experiencing seizures and assist people with transferring to/from wheelchairs

路 Must be capable of pushing, pulling, lifting up to 60 lbs.聽

路 Must be able to physically assist people who are experiencing aggressive or assaultive behavior.


PRIMARY DUTIES:

路 Manage and supervise up to but not limited to 40 employees depending on size of case load

  • Case load can grow up to but is not limited to 10 (case load subject to change upon employer discretion at any time)

路 Be part of 24/7 on call rotation, expected to be available to customers and staff during week on-call, respond and write reports within a two-hour window

路 Pro- actively utilize constructive feedback, corrective action and corresponding documentation when developing Support Coaches and all team members

路 Assure IHSS is maximized and managed for each individual served

路 Assure Section 8 and all other social assistance is applied for and maintained

路 Train, Develop, Delegate and provide feedback to employees in a way that assures outcomes are met for individuals while also meeting company financial and philosophical objectives and exceeding Regional Center requirements.

路 Train Support Coaches

路 Assure that the individuals served are provided with assistance in all aspects of their daily living and that they receive supports in all areas needed to live a full active life of their choice.

路 In conjunction with the individual, assure implementation, documentation and follow through on all aspects of the IPP (Individual Program Plan) for people served.

路 Assure Supported Living Plans, Behavior Intervention Strategies and ISPs are in place and current for all individuals served and that accurate data is kept addressing those plans.

路 Assure Annual and Quarterly reports are submitted to Director and Regional Center for all individuals on caseload.

路 Assure Submission of Special Incident Reports in a timely fashion and in accordance to regional center requirements

路 Act as an advocate for people served.

路 Assure staff coverage is always present at the specified level of support

路 Act as primary liaison/consultant to individual鈥檚 team.聽

路 Assure that the human and civil rights of persons served are being respected at all times.聽

路 Review and prepare administrative reports and documents as necessary.

SECONDARY DUTIES:

路 Assist Director in development of Capitol and Operational Budgets

路 Assist in development of Strategic Plan

路 Train Support Coaches聽

路 Responsible for assuring that people served are happy with their services and receiving all provisions as agreed in their IPP

路 Responsible for on going Quality Audits and interviews

路 Responsible for carrying a phone and responding to calls within 5 minutes.

路 Assure thorough assessments are completed when challenging behaviors emerge.

路 Provide immediate on-site response in emergencies.

路 Communicate with Regional Center representatives, Parents/Guardians at a frequency desired by the respective individuals

路 Act as Support Coach when needed

路 Attend and participate in Individual Program Plan and interim meetings.

路 In consultation with Director and Executive Director develop policies that address behavior intervention to assure that Lynn & Darla provides an environment where the least restrictive approaches are used in behavior management.

路 Review incident reports for trends and submit in monthly report to
Director

路 Attend training and conferences to assure that Lynn & Darla acts as leader in the field聽

路 Attend all Regional Center audits/IHSS interviews to assure that auditors
understand implementation of supports.

路 Assure that Support Coaches complete all duties as specified in the Support Coach job description.

路 Assure that Associate Coordinators complete all duties as specified in the Associate Coordinator job description.

路 Assume responsibility for all other duties as assigned by Director or Executive Director.

Requirements

QUALIFICATIONS:

Education: B.A. in Psychology or related field preferred or 4 years experience in the field of Developmental Disabilities.聽

Experience: 2 years direct support experience with people with behavior challenges, medical involvement or psychiatric diagnosis.聽

1 year experience coordinating services for people with developmental disabilities (QMRP, IPC, Community Service Coordinator) preferred

1 year experience in a Supervisory position preferred.

Certifications: 聽Must have and maintain current CPR/First Aid Certification and present upon hire.

Other: Must obtain the following certifications within the first 30 days of employment:聽

Must obtain the following within the first 90 days of employment:

1. 12 hours of Employee Development and Time management training.聽
