At The Meadows of Napa Valley, an upscale Continuing Care Retirement Community, we’re looking for a compassionate and experienced Registered Nurse to act as our Resident Transition Manager to guide residents through transitions in care and ensure their health and happiness every step of the way.

What You’ll Do
- Coordinate smooth resident transitions between levels of care and oversee new admissions and discharges at our Health Center.
- Build relationships with hospitals, physicians, families, and managed care partners.
- Support residents with individualized care planning and advocate for their well-being.
- Collaborate with interdisciplinary teams and lead nursing staff on the floor as needed.
- Ensure regulatory compliance and the highest quality of care for all residents.
What We’re Looking For
- LVN or RN license (required)
- 2+ years of nursing experience in skilled nursing, assisted living, or healthcare settings (5+ preferred)
- Strong communication, leadership, and organizational skills
- Knowledge of MDS, care planning, and regulatory standards
- Passion for empowering older adults to thrive
Schedule
- Monday – Friday
Wage Range
- $96,000 - $120,000/ annually DOE & license typeÂ
