The Admissions Manager is the primary contact between The Chateau & referral sources. The Admissions Manager reviews referrals and coordinates the admission of residents to the Chateau. This position will also prepare admissions reports by collecting, analyzing, and summarizing data and trends. Organizes patient records and documentation. Drives census growth through relationship-building and strategic outreach while upholding the facility’s standards of care.


Contributions:

  • Conducts job responsibilities in accordance with Moorings Park policies & procedures, applicable federal & state laws, & applicable professional standards.
  • Reviews Residents medical charts in collaboration with the facility leadership & provides case managers with a timely response to acceptance/denial of Residents into our SNF for rehab or private pay. 
  • Communicates with & manage professional referral sources to insure the smooth coordination of inquiry to admission at The Chateau. 
  • Responds to inquiry calls from hospitals, families, physicians & referral sources, gives tours, & handles admission process. Aware of bed availability at all times. Ensures responses to inquiries are handled properly, timely, & with appropriate follow-up. Must obtain information to respond to calls from resources outside the facility.
  • Coordinates room placement & ensures room readiness for new admissions & in-house transfers.
  • Alerts appropriate department heads & facility staff of projected changes (e.g., admissions, room moves, & discharges) by attending & reporting at morning “Stand Up” meeting, as well as providing necessary e-mail & telephone updates. Maintains close working relationship with the Director of Nursing and Administrator of The Chateau to ensure appropriate & efficient decisions about prospective admissions.
  • Enters all Resident medical/personal information into EMR to create a working chart for Resident admission & ensures new admission information & face sheet provided to the nurses in anticipation of new admission.
  • Maintains knowledge of federal & state regulations, as well as reimbursement guidelines (Medicare). Maintains working knowledge of insurance, runs eligibility, obtains authorization for admission from insurances (non- Medicare) provides updates for continued length of stay, & ensures documents received by insurance companies.
  • Runs & scans Resident’s HIQA into EMR to ensure Resident has proper insurance coverage for admission. May assist Transition planner to ensure that all admissions documents are signed by residents & residents are provided a copy & obtains advance deposits from residents transferring or being admitted to a private pay status
  • Prepares & maintains census for The Chateau.
  • Maintains principals of confidentiality & Health Insurance Portability & Accountability Act (HIPAA) regulations & understands the legal/regulatory requirements related to the release of information.
  • Participates in the facilities monthly triple check meeting. 
  • Collaborates with the finance department ensuring that resident billing and demographic information is correct in the EMR. 


Job Requirements:

  • Excellent oral and written communication skills in the English language required
  • During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
  • Highschool diploma or equivalent required
  • 3 + years of healthcare admission experience required
