Patient Navigation

路 Coordinate the patient journey from referral through completion of diagnostic testing and follow-up care.

路 Ensure patients are scheduled for recommended imaging and follow-up appointments before leaving the clinic whenever possible.

路 Serve as the primary point of contact for non-clinical patient coordination.

路 Monitor patients requiring cardiac CT, cardiac MRI, stress echocardiograms, PET, SPECT, and other cardiovascular diagnostic procedures.

路 Track patients throughout the imaging process to ensure completion.

Imaging Coordination

路 Review imaging work queues daily.

路 Coordinate imaging appointments with Radiology and Cardiology.

路 Ensure imaging orders contain all required documentation.

路 Coordinate with authorization staff to ensure insurance approvals are completed.

路 Escalate scheduling or authorization delays.

路 Coordinate rescheduling of cancelled or missed imaging appointments.

路 Maintain tracking lists for outstanding imaging studies.

路 Maintain and actively manage imaging waitlists to maximize imaging capacity.

路 Review daily provider schedules to identify patients needing imaging or follow-up before leaving clinic.

路 Coordinate same-day scheduling opportunities to fill cancellations.

路 Monitor imaging scheduling turnaround times.

Follow-up Coordination

路 Schedule provider-recommended follow-up appointments.

路 Monitor follow-up work queues to ensure patients are not lost to follow-up.

路 Contact patients who miss appointments, cancel appointments, have incomplete imaging, require repeat testing, or need ED/hospital follow-up.

路 Track outstanding imaging orders and coordinate completion.

路 Monitor referral leakage and coordinate appropriate follow-up.

路 Coordinate surveillance and repeat imaging based on provider recommendations.

Patient Communication

路 Contact patients regarding appointments, preparation requirements, scheduling, and follow-up.

路 Reinforce procedure instructions provided by nursing staff.

路 Answer non-clinical questions and route clinical concerns appropriately.

路 Encourage adherence to care plans and scheduled testing.

Coordination with Clinical Staff

路 Collaborate with physicians and nursing staff to ensure patients receive pre-procedure instructions, medication instructions, laboratory requirements, and preparation materials.

路 Notify nursing staff of patients requiring clinical outreach or education.

Care Gap Management

路 Manage referral, imaging, recall, ED follow-up, hospital discharge, missed appointment, incidental finding, surveillance imaging, and waitlist work queues.

路 Ensure timely patient outreach and appointment completion.

Program Growth and Service Line Development

路 Increase completion rates for advanced cardiac imaging.

路 Reduce delays from provider order to completed imaging.

路 Reduce no-show and cancellation rates.

路 Maximize imaging utilization through proactive scheduling.

路 Monitor referral conversion and imaging volumes.

路 Support outreach to referring providers and new cardiovascular service initiatives.

路 Support population health and surveillance imaging programs.

Reporting and Performance Monitoring

路 Maintain dashboards and tracking tools for outstanding imaging orders, referrals, follow-up appointments, and work queues.

路 Provide weekly and monthly reports to clinic leadership and providers on outstanding tasks, turnaround times, completion rates, no-shows, referral conversion, waitlist utilization, and operational barriers.

路 Present findings during operational, provider, and quality meetings.

路 Recommend workflow improvements based on trends and performance data.

Documentation

路 Document all patient outreach and coordination activities in the electronic medical record.

路 Maintain tracking spreadsheets, dashboards, and operational reports.

Quality Improvement

路 Participate in quality improvement initiatives, workflow redesign, and departmental meetings.

Minimum Qualifications

路 Associate's degree preferred.

路 Three years of healthcare experience preferred.

路 Experience in cardiology, ambulatory operations, patient access, scheduling, referral coordination, or care coordination preferred.

路 Experience with Cerner or comparable electronic medical record preferred.

Knowledge, Skills and Abilities

路 Excellent organizational and communication skills.

路 Strong customer service and interpersonal skills.

路 Knowledge of medical terminology, referral management, and insurance authorization processes.

路 Ability to manage multiple priorities independently.

路 Proficiency with Microsoft Office and EMR systems.

Additional Responsibilities

路 Participate in departmental meetings and special projects.

路 Cross-train to support ambulatory cardiology operations as needed.

路 Perform other duties as assigned.
