Intake Coordinator – Independence Home Health LLC
Independence Home Health LLC is seeking a dependable and detail-oriented Intake Coordinator to join our team. The ideal candidate is organized, adaptable, and committed to providing excellent service to patients and healthcare partners.
Responsibilities
- Manage and coordinate patient intake and referrals
- Communicate with patients, families, physicians, and staff to obtain necessary information
- Review and verify documentation for accuracy and completeness
- Maintain organized records and follow compliance guidelines
- Qualifications
- High school graduate and at least 18 years of age
- Minimum two (2) years of work experience, preferably in a healthcare setting
- Detail-oriented with strong communication and customer service skills
- Knowledge of medical terminology, insurance processing, and prior authorizations preferred
- Familiarity with home care regulations and community resources
- Proficient in Microsoft Word and Excel
- Strong organizational, problem-solving, and multitasking skills
- Home Health Aide (HHA) or Certified Nursing Assistant (CNA), or willingness to obtain certification
- Valid driver’s license and insured vehicle in accordance with agency requirements
