JOB SUMMARY:Ā Ā
The Learning and Development Lead, Care Management is responsible for designing, implementing, and evaluating training programs to enhance competencies and professional growth within MHCās Care Management team. This role collaborates with leadership to assess learning needs, develop educational strategies, and ensure training aligns with best practices, regulatory requirements, and value-based care principles. The Learning and Development Lead will oversee training operations, lead cross-functional initiatives, and drive continuous learning to improve patient outcomes and care coordination.Ā

Training Program Development & Implementation:Ā
Develop and implement comprehensive training programs to enhance care management competencies and support professional development.Ā
Ensure all training programs align with evidence-based care management practices, value-based care models, and regulatory requirements.Ā
Learning Needs Assessment & Strategy:Ā
Collaborate with leadership to assess staff learning needs and develop targeted training strategies.Ā
Evaluate the effectiveness of training initiatives through feedback, assessments, and performance metrics.Ā
Operational Management & Compliance:Ā
Oversee the administration and delivery of training programs, including orientation, continuing education, and specialized care management skill development.Ā
Ensure training content meets accreditation standards, regulatory compliance, and organizational policies.Ā
Cross-Functional Collaboration & Leadership:Ā
Work closely with the Director of Value-Based Care, care management teams, and other departments to align training efforts with organizational goals.Ā
Lead training sessions, workshops, and competency evaluations to enhance staff knowledge and skills.Ā
Performance Improvement & Innovation:Ā
Monitor and analyze training outcomes to identify opportunities for process improvement and curriculum enhancement.Ā

Bachelorās degree in Healthcare Administration, Public Health, Nursing, Social Work, or a related field required; Masterās degree preferred.Ā
Minimum of 3-5 years of experience in care management, value-based care, or healthcare training roles.Ā
Strong understanding of care management principles, value-based care models, and adult learning methodologies.Ā
Experience in developing and implementing training programs in a healthcare setting preferred.Ā
Excellent communication, leadership, and problem-solving skills.Ā
Strong understanding ofclinical operations and healthcare quality metrics.Ā
Proven ability to analyze healthcare data, identify trends, and implement performance improvement initiatives.Ā
