Intake Coordinator/Social Worker – Independence Home Health LLC
Independence Home Health LLC is seeking a dependable and detail-oriented Intake Coordinator /Social Worker to join our team. The ideal candidate is organized, adaptable, and committed to providing excellent service to patients and healthcare partners.

Responsibilities

  • Manage and coordinate patient intake and referrals
  • Communicate with patients, families, physicians, and staff to obtain necessary information
  • Review and verify documentation for accuracy and completeness
  • Maintain organized records and follow compliance guidelines

  • Qualifications
  • High school graduate and at least 18 years of age
  • Minimum two (2) years of work experience, preferably in a healthcare setting
  • Detail-oriented with strong communication and customer service skills
  • Knowledge of medical terminology, insurance processing, and prior authorizations preferred
  • Familiarity with home care regulations and community resources
  • Proficient in Microsoft Word and Excel
  • Strong organizational, problem-solving, and multitasking skills
  • Home Health Aide (HHA) or Certified Nursing Assistant (CNA), or willingness to obtain certification
  • Valid driver’s license and insured vehicle in accordance with agency requirements
