Job Description:

Baptist Health is looking for a Home Infusion RN Hospital Liaison to join our Home Infusion team at Baptist Health Floyd!

Monday -Friday from 9am -5:30pm.

On call rotating weekends for new referrals leaving the hospital.

  • Advocate for patients and families, ensuring their home infusion needs are properly identified and met.

  • Educate patients and their families on infusion procedures, medication administration, and how to manage potential side effects.

  • Provide guidance on maintaining a sterile environment and proper use of infusion equipment.

  • Educate patients and their families on managing illnesses or injuries, including post-discharge care and chronic condition management.

  • Maintain accurate, timely, and detailed documentation of all patient care activities in compliance with department and regulatory standards.

  • Regular and reliable attendance is an essential function of this role. Healthcare workers must be present and punctual for all scheduled shifts to ensure the delivery of high-quality patient care.

  • Provide education to hospital staff about home infusion services.

  • Collaborate with the healthcare team, including physicians, pharmacists, and case managers, to develop and implement individualized care plans.

  • Serve as the primary contact between home infusion and the local health care facility.

  • Coordinate patient referrals from hospitals to home infusion, ensuring smooth transitions and timely service.

  • Participate in quality improvement initiatives and contribute to the development of best practices for home health care.

  • Meet established productivity standards by managing caseloads efficiently document appropriately within designated timeframes.

  • Ensure timely and accurate submission of patient care notes, reports, and other required documentation to meet department and regulatory deadlines.

  • Balance patient care needs with productivity goals, ensuring high-quality care without compromising efficiency.

  • Coordinate with the home infusion pharmacy regarding patient care and insurance coverage.

Minimum Requirements

  • Associate's degree in nursing, BSN preferred.

  • Graduate of an accredited nursing program.

  • 1 year of job-related experience.

  • Experience with Central Lines, PICC Lines, Ports, and Peripheral IV’s.

  • ​Must have valid Indiana RN license.

  • BLS issued by American Heart Association.

  • Valid driver's license.
