SIGN ON BONUS: $20,000   Long Term Care Houses - Days: Monday - Friday 7a-3pm, Holidays and Weekends OFF! Salaried Position!   Duties: Responsible for the overall functioning of the houses, 50 long-term care beds, over 24hr. period in conjunction with the Nursing Supervisors.   Is responsible for the daily activities of each house and delegates duties to house staff to assure continuity of care and services to residents over 24-hr. period.   Monitor house staff for proper implementation of departmental policies and procedures and identify areas in need of training - communicating such needs and making appropriate employee referrals to Education and Training as needed.   Supervises the scheduling of employee's time for house for day shift to maintain adequate nursing coverage for house.   Provides input for completion of probationary performance evaluations for CNA's and Nurses.   Regularly inspects houses to ensure compliance with federal, state and local standards.   Contacts ancillary departments when issues arise related to provision of services either directly or environmentally in need of attention.   Acts as liaison between medical staff and families regarding patient care issues.   Determines need for notifications of physician, families and/or administration of events occurring on shift.   Attends weekly care plan meetings for residents and/or assigns appropriate staff to attend.  Ensures completion of Fall Risk Assessments and Braden Scales prior to review (as well as Braden Scale completion per admission policy).   Assigns staff to in-service training and assures compliance.   Responsible for facility supervisory shift coverage as deemed necessary by facility needs/D.O.N.   Actively participates in: Quality Assurance Performance Improvement Committee Safety Committee Leadership Management Meeting Morning Report Skin Care Meetings   Access to entire medical record of all patient's/resident's.   Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and applicable governing laws.   Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.   Performs any other duties assigned by the D.O.N.  QualificationsGraduate of an approved/accredited School of Nursing and currently licensed as a Registered Professional Nurse in New York State. Bachelor's degree in nursing preferred. Previous leadership experience with at least two years of supervisory experience in the nursing field. Knowledge of Federal and State Regulations. Ability to organize, plan and implement nursing service policies and procedures. Displays an active interest in pursuing continuing education to keep knowledge and skills current. Knowledge of completion Minimum Data Set (MDS) preferred. Demonstrates competency by completing an annual job specific skills checklist.   Additional Information: Charles T.
