Title: Nurse Practitioner – Street Medicine
Department: Street Medicine Team
Reports To: Clinical Director, Street Medicine / Chief of Medicine

Summary

The Nurse Practitioner provides medical care to individuals served by the Street Medicine Team. The care environment ranges from working out of a backpack in the woods and in the street to working in our street medicine clinics. Responsibilities include primary and urgent care, encompassing assessment, diagnosis, treatment, and education. The provider practices with independence, sound clinical judgment, and adherence to evidence-based standards under the supervision of the Clinical Director and Chief of Medicine.

Essential Duties and Responsibilities

  • Provides evidence-based medical and surgical care using sound clinical judgment
  • Maintains confidentiality of patient, staff, and program information
  • Makes appropriate referrals for acute and chronic specialty follow-up
  • Communicates professionally with nursing, navigators, management, and external agencies
  • Educates patients on preventive health and chronic disease management
  • Treats team members with respect and support
  • Sets and maintains clear professional boundaries to support work/life balance
  • Focuses on clinical care while handing off navigation and case-management responsibilities to appropriate staff
  • Maintains a clean and safe working environment and follows infection-control protocols
  • Completes timely and accurate documentation and coding
  • Participate in meetings and required training
  • Performs other related duties as assigned

Qualifications

Education: Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school. CPR Certification required. 

Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.
