PurposeÂ
Under the direction of the Program Manager of Case Management in New Haven, the Case Manager provides case management to homeless residents, networks with the Greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems and collaborates with clinicians and other service providers as well as other Columbus House Case Managers to empower clients to end their homelessness, works with GNHCAN to ensure maximum bed capacity within all Shelter Programs, complete and correct data quality reports related to enrollments and discharges, and provide case manage services to the New Beginnings Program, Shelter Veterans Program and On The Move Veterans Program.

Duties and ResponsibilitiesÂ
- Provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, and or substance abuse servicesÂ
- Provide transportation to appointments for clientsÂ
- Coordinate and collaborate with other agencies, funders, and social service providers
- Oversee intake, departures and daily roster and programsÂ
- Attend weekly and monthly agency, program and network meetingsÂ
- Monitor all cases assigned and complete all documentation/charting required by Columbus House and its relevant funders
- Maintain and update all required charting and notes
- Enter and maintain all client charts accurately and timely into the Homeless Management Information System Database Case WorthyÂ
- Other related duties assigned in support of a 20-client case load
- Collaborate with GNHCAN, CMHC and Errera Center to maximize occupancy of Shelter Programs.
- Work closely with Quality Department and Director to complete error reports and maintain accurate admissions and discharges for Shelter Programs.
Qualifications
- Bachelor’s degree in human services required with a minimum of One years’ experience working in human services or related fields
- Excellent organizational and time management skills with a deadline-oriented mindset
- Strong written and oral communication skills employing the English language
- Proficiency is Microsoft Office and other common electronic applications
- Able to effectively use the local network of substance abuse, mental health, and social services providers
- Familiar with criminal justice and able to engage, manage, motivate, and support
- Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics
- Complete knowledge base of all aspects of the delivery of person-centered case management
- Able to meet all documentation requirements
- Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma informed care and motivational interviewing
- Valid drivers’ license with a good driving history
Supervised by: Program Manager of Case Management
Schedule: 32 hours per week; Monday & Wednesday 9 AM–5 PM, Tuesday 10 AM–6 PM, Thursday 8 AM–4 PM
Salary range:Â $25.50 - $27.50
