Assists with assessment, intervention, and follow-up of referred patients. Provides case management including continuum care planning and advocacy. Serves as department community resource development specialist and as a liaison between select patients of all ages and community service systems. Performs other job duties as required.

Qualifications
EDUCATION

Bachelor of Social Work degree from a program accredited by the Council on Social Work Education or equivalent bachelor's degree with experience in working in a related helping profession or with community agencies.

EXPERIENCE
Â
May be a new graduate. Preference is given if experienced in health or mental health social work or related helping profession.

SPECIAL SKILLS
Â
Competency in caring for patients treated at the Hospital regardless of age, disability, sexual preference, gender, ethnicity, or diagnosis. Computer literacy.
