FlexStaff is seeking a Registered Nurse (RN) with experience in home care (CHHA) to work full-time as a Nurse Instructor for our client, a certified home care agency, located in Brooklyn, NY. 

 

Schedule: Monday - Friday 8:30 am - 5:00 pm.

Permanent Position.

 

In this role you will be taking referrals and processing appropriate home care services; as well as maintaining open lines of communication with referral sources and within the company departments. 

 

Key Position Requirements:

  • License: Active NYS RN License.
  • Experience: Minimum of 2 years of nursing or intake experience in a licensed home care agency (CHHA).
  • Knowledge of Allscripts a MUST
  • EMR knowledge: HomeCare HomeBase (HCHB) or Careport
  • Compensation: $100,000 annually

 

Responsibilities include, but not limited to:

  • Screen referrals for clinical appropriateness and payer eligibility 

  • Triage new referrals and identify urgent clinical needs 

  • Review medical records, hospital summaries, and current medications 

  • Verify and obtain physician orders and prior authorizations as needed 

  • Complete timely intake documentation in the EMR 

  • Coordinate admission scheduling with patient/family, clinicians, and physicians 

  • Communicate referral details to the interdisciplinary team 

  • Educate patient/family on home health services, visit expectations, and patient rights 

  • Identify and escalate safety risks, infection control issues, or barriers to care 

  • Arrange or schedule initial assessments (telephonic or in-person) 

  • Maintain compliance with Medicare, state, and agency admission requirements 

  • Track referral outcomes, denials, and report trends to management 

  • Provide telephonic triage or on-call handoff for intake issues (if applicable) 

  • Train/support intake staff on screening and documentation standards
