Summary:
As the Nurse Practitioner Supervisor, you will work closely with community primary care physicians to provide top-tier oversight and coordination of care for our participants. Your leadership will ensure personalized, holistic care that promotes independence, wellness, and improved quality of life. You’ll serve as a trusted liaison among participants, physicians, and our interdisciplinary team—developing care plans, monitoring health status, and facilitating preventive services.
- Lead and supervise a dedicated team of Nurse Practitioners and clinical staff.
- Play a key role in delivering comprehensive wellness assessments and urgent care.
- Be the voice of excellence in care coordination, ensuring seamless transitions and optimal health outcomes.
- Engage in meaningful work with a diverse, frail, and elderly population.

Responsibilities:
- Manage a PACE site, overseeing all participant care activities.
- Supervise clinical staff, providing mentorship and performance feedback.
- Conduct annual wellness assessments and manage complex cases with a focus on multiple comorbidities.
- Collaborate with community physicians to develop and adjust personalized care plans.
- Participate in interdisciplinary team meetings, represent CPCP, and coordinate care strategies.
- Lead efforts in clinical documentation, quality improvement, and risk adjustment.
- Perform home visits, provide emergency coverage, and support care transitions.
- Ensure compliance with all healthcare regulations and maintain the highest standards of confidentiality.

Qualifications:
- Graduate of an approved Nurse Practitioner program; NYS licensure required.
- Minimum of 2 years in clinical nursing, with at least 1 year working with frail or elderly populations.
- Board Certified (AANP or ANCC), NY Registered Nurse and Nurse Practitioner licenses, DEA certification.
- Strong communication, leadership, and analytical skills.
- Ability to manage multiple projects in a fast-paced environment.
- Proficiency in Microsoft Office.
