Qualifications
The Case Manager serves as provider of patient-centered services that promote coordinated, effective treatment by facilitating the communication exchange between patient/family, physician/treatment team and payor and managing timely transitions through the phases of rehabilitation.
- Communicate patient progress and continued needs with family/guardian/referrals source and payor source throughout treatment process
- Coordinates patient’s transition outside the facility in a manner which assures continuity of care post discharge
- Coordinates patient centered treatment through the interdisciplinary team.
