Job Description

The Advanced Practice Registered Nurse will provide psychiatric medication management alongside a multi-disciplinary, community based team serving individuals who have severe and persistent mental illnesses.


Minimum Requirements

  • Advanced clinical license as a Nurse Practitioner (required)
  • Be over 18 years of age (required)
  • No restrictions or adverse actions that would disqualify for Medicaid or other billing (required)
  • Possess a valid driver’s license (required)

  • Promotes recovery, independence, and personal choice for participants.


Job Duties / Skills Required 

  • Practices on a multidisciplinary treatment team.
  • Provides behavioral health care services including assessment, diagnosis, treatment.
  • Prescribes and manages medications.
  • Assists clients in identifying strengths, needs, and realistic goals to develop a treatment plan.
  • Accurately identifies risk and responds appropriately, and provides consultation to peers regarding high-risk situations and appropriate interventions.
  • Provides clinic and community-based services as appropriate.
  • Provides effective, research-based recovery based services.
  • Establishes service plans and treatment goals based on accurate assessment and diagnosis.
  • Completes clinical documentation accurately and at the time of the services.
  • Fully utilizes approved electronic health records.
  • Collects data in the required format to comply with all funding sources, accreditation, or other regulatory agencies.
  • Coordinates referrals to other agencies and specialists as indicated by patient need.
  • Maintains a clear driving record in order to be able to drive company vehicles when necessary. 
  • Attends weekly and/or monthly staff meetings.
  • May assist with clinical consultation, clinical review, or auditing of clinical records.
  • Provides direct clinical services at the advanced, independent practitioner level using approved interventions and completing necessary documentation to receive payment from insurance companies, Medicaid, Medicare, etc.
  • Supervises other staff as directed.
  • Performs any other duties as assigned by Supervising Physician or Program Director.
