Your EverydayĀ 

Collaborate with multidisciplinary team members to ensureĀ appropriate directĀ patient care and implementation and documentation of care plan progress.Ā 

Maintain andĀ advocateĀ a safe environment for everyone, report toĀ appropriate careĀ team members, andĀ initiateĀ corrective actions.Ā Ā 

Monitor and engage with your patients, families, andĀ appropriate careĀ team members concerningĀ significant events, patient condition changes, or patient questions and concerns.Ā 

Conduct rounds, answer callĀ lightsĀ and ensure adequateĀ assistanceĀ with meals, personal and oral hygiene needs.Ā 

Participate in the implementation of the plan of care.Ā 

Identify problems, concerns, changes in condition and report them immediately to appropriate care team personnel.Ā 

Work with nursing staff, physicians, families, consultantĀ personnelĀ and ancillary service providers as directed by nursingĀ supervisor.Ā 

Ensure continuity of care, consistent communication, and an exceptional experience for every patient.Ā 

Follow established performance standards and duties according to policies and procedures and advise and consult with supervisor when unable to complete assignments or perform tasks promptly, effectively, or safely.Ā 

Initiate patient and family education usingĀ appropriate resourcesĀ and continuum of care services.Ā 

Contribute to the team byĀ assisting others without being asked.Ā

The Must-HavesĀ

Minimum:Ā 

High School Diploma,Ā GEDĀ or equivalent.Ā Ā 

Louisiana Department of Public Health Certified Nurse Assistant license.Ā 

American Heart Association BLS certification.Ā
