St. Bernard Nursing & Rehabilitation Center is seeking a Social Services Director to join our team and make a meaningful difference in the lives of our residents and their families.

As our Social Services Director, you’ll play a key role in supporting residents throughout their stay—from admission and adjustment to care planning and discharge. You’ll work closely with residents, families, nursing staff, providers, and other members of the interdisciplinary team to ensure each resident’s social, emotional, and psychological needs are met.

What You’ll Do

• Complete social histories, assessments, and individualized care plans for residents upon admission.

• Identify residents’ social, emotional, and psychological needs and develop appropriate goals and interventions.

• Participate in interdisciplinary Care Plan Meetings and collaborate with the care team to support resident outcomes.

• Maintain timely and accurate documentation, including progress notes and required social services records.

• Assist residents and families with discharge planning, placement, and transition of care.

• Support residents and families through difficult transitions and provide counseling and resources as appropriate.

• Organize and facilitate family meetings and groups to promote communication, education, and support.

• Participate in the pre-admission screening process and help prospective residents and families understand facility services.

• Help new residents adjust to the long-term care environment and promote a positive experience from admission forward.

• Educate residents, families, and the community on topics including aging, resident rights, available services, and related resources.

• Maintain department records and assist with managing departmental supplies and budget needs.

• Ensure compliance with company policies as well as applicable state and federal regulations.

What We’re Looking For

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree in Social Work (MSW) where required.

• Licensed Social Worker required.

• At least 2 years of social work experience.

• Healthcare experience strongly preferred, particularly long-term care/skilled nursing.

• Knowledge of applicable state and federal regulations and long-term care guidelines.

• Strong communication, documentation, assessment, and social work skills.

• Comfortable using basic computer technology and electronic documentation systems.

• Ability to communicate effectively in English, both verbally and in writing.

• Compassionate, professional, and able to build positive relationships with residents, families, coworkers, and community partners.

• Ability to work collaboratively as part of an interdisciplinary team.
