Job Description

Position Summary:

To support the Clinical Pharmacy team workflow and provide direct telephonic patient care to support overall quality of Comprehensive Medication Management services and Pharmacy related quality metrics for our high risk attributed patient population.

This role will work remotely 3 days per week and in the New Providence, NJ office every Tuesday and Wednesday.

Essential Job functions:

The Clinical Pharmacy Technician will be expected to demonstrate ability to perform the following:

  • Interact with all levels of medical staff to discuss relevant patient information as warranted.

  • Conduct chart review and patient interviews to identify patient specific reasons for poor medication adherence and resolve identified problem per protocol.

  • Perform medication reconciliation for patients recently discharged from the hospital or as requested by the Clinical Pharmacist per protocol. Record medication information into the "Meds" in the electronic health record and update existing information as needed to ensure accuracy of the patient home medication list.

  • Gather data and generate reports to improve clinical pharmacy quality metrics.

  • Document patient cases in electronic medical record effectively.

  • Appropriately identify situations for referral to Clinical Pharmacist (i.e. medication adverse event, medication ineffective, drug-drug interaction, lab test indicated).

  • Reinforce patient self-management of medications through provision of education on proper medication administration, medication adherence, storage, and drug disposal.

  • Prepare and manage patient education materials/handouts.

  • Make outreach to patients identified from payer medication adherence-related reports.

  • Participate in training and orientation of new employees as needed.

  • Comply with the Summit Health Management incident reporting policy and procedures.

  • Adhere to all Summit Health Management training and accomplishments as required per policy.

  • Provide other job duties as assigned.

  • Report issues and concerns to manager in a timely manner.

  • Based on chart review or patient interview, identify patient specific reasons for poor medication adherence and take appropriate action to resolve problem.

Education, Certification, Computer and Training Requirements:

  • Associate’s or Bachelor’s Degree

  • New Jersey Board of Pharmacy Technician registration

  • Certified Pharmacy Technician (CPhT) with PTCB

  • Minimum 3 years of retail Pharmacy practice

  • Ability to communicate in English, both orally and in writing

  • Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail)

  • Standard Office Technology in a Window based environment

  • BLS Certification

  • Valid License

Pay Range: $23.56 - $28.99 Hourly

The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
