Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist (PT) to join our growing therapy team at our senior living community in New Richmond, WI and its surrounding communities.  This is a great opportunity for a dedicated and flexible PT looking to make a meaningful impact, with the potential to grow into a consistent part-time role.

Your Responsibilities

Evaluate and treat patients in a skilled nursing setting

Develop individualized care plans to improve mobility, strength, and function

Collaborate with the interdisciplinary team to ensure continuity of care

Document patient progress in accordance with regulatory and company standards
What You Bring

Active Wisconsin Physical Therapist license

Strong clinical, communication, and organizational skills

Qualifications
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
