Sonic Summary

The Medical Case Manager is responsible for referring PATH adult program patients to necessary health and psychosocial services and entitlements.
Candidates should have a High School diploma or GED, with an Associate or Bachelor’s Degree highly preferred.
A minimum of two years of experience in HIV treatment, medical case management, or health education is required.
Strong written and oral communication skills are essential, along with preferred experience using an Electronic Medical Record.
Bilingual proficiency in English/Spanish or English/Creole is preferred for this position, which offers a competitive salary.

The Medical Case Manager ensures that all PATH adult program patients are referred to appropriate health and psychosocial services, as well as for entitlements.

Qualifications

High School or GED
Associate or Bachelor’s Degree – Highly Preferred
At least two (2) years of experience in HIV treatment, medical case management and/or health education
Must have strong written and oral communication skills.
Prior experience using an Electronic Medical Record preferred.
Bilingual English/Spanish or English/Creole preferred
