Associate Director, Health Services at JASA
As the Associate Director of Health Services, you will provide strategic leadership and operational supervision across JASA’s core healthcare teams: Care Transitions, Medication Adherence, Social Determinants of Health (SDOH), Palliative Care, and Health Home. This senior role bridges clinical oversight, partner management, and modern operational efficiency—utilizing data analysis, workflow automation, and process improvement to elevate client outcomes and scale health programs across NYC.

What You’ll Be Doing
You will balance clinical staff supervision and weekly partner reporting with strategic workflow optimization and business development.
Key Responsibilities:
Care Transitions Leadership:Â Supervise, recruit, and onboard Care Transitions Specialists (CTSs); develop clinical trainings, monitor performance metrics, and provide direct service coverage as needed.
Medication Adherence Management:Â Direct daily operations, task assignments, and client prioritization; prepare weekly partner reports and lead partner meetings to address adherence barriers for high-risk members.
Process Improvement & Automation: Identify efficiency initiatives across Health Services programs—collaborating with leadership to streamline manual workflows, increase data accuracy, and integrate smart automation tools.
Program Oversight & Integration:Â Oversee Health Home and Palliative Care operations, ensuring achievement of monthly targets while promoting service integration across all health programs.
Partner & Business Development:Â Strengthen relationships with health plans and hospital systems; prepare presentations, lead partner reviews, and support new program initiatives.
Evaluation & Strategy:Â Support the Senior Director of Health Services in overall program management, quality monitoring, data analysis, and the launch of new clinical initiatives.
What You’ll Need
Education: International Medical Graduate (IMG), Master’s degree in a health-related field, or equivalent combination of education and relevant professional experience.
Language Skills: Bilingual English/Spanish required (fluent in reading, writing, and speaking).
Leadership Experience: Minimum of 3 years of managerial and leadership experience with a proven track record in staff training and knowledge transfer.
Technical & Automation Skills: Proficiency in MS Office, Google Workspace, and EHR systems. Experience with process improvement, workflow automation, Python, AI applications, or data analysis techniques is highly desirable.
Sector Expertise:Â Hospital discharge planning, care coordination, or health system experience preferred.
Travel & Physical Requirements:Â Ability to travel to clients' homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
