We are currently seeking a Care Coordination Programs Manager to work at our Manhattan location.

 

Position Summary: The Care Coordination Programs Manager is responsible for oversight of the grant funded programs in the Department of Support Services. The major programs are Care Coordination (CC), Be Into Health (BEM), PrEP/PEP Navigation.


The major duties of the position include, but are not limited to:

Program Management & Compliance –

  • Ensure overall compliance of programs assigned under the Department of Support Services such as service level, quality, budget, and reports.
  • Assist the Director of Support Services with grants management including to create the assigned projects under the Department of Support Services on revenue projection and expense budget.
  • Monitor and ensure staff maintain accurate patients and program related records on the services and activities such as enrollment, care plans, patient’s interactions in funder assigned and agency required designated database to meet reporting and documentation requirements.
  • Assist the Director of Support Services by monitoring program deliverables and meet milestones.
  • Responsible for ensuring the program operates within services quota established; adapt strategies in changing environment to meet target goal.
  • Other activities as assigned.

People Management & Training –

  • Monitor team’s performance and ensure meeting the service goals and quota set by Apicha Department of Support Services.
  • Ensure the program staff follows Apicha CHC policy and procedure as required.
  • Work with the staff to ensure the program patients meets the program eligibilities and are compliance with program guidelines.
  • Recruit and train the program staff including supervision and performance review.


 

Quality Assurance & Improvement –

  • Perform quality assurance and quality improvement activities.
  • Participate/collaborate with internal and external entities on quality improvement activities.
  • Collects, prepares, and reports of services and activities completed in a timely manner to internal and external entities, as required.
  • Provide aggregate data and assessment information to internal and external entities, as needed.

 

Collaboration & Services –

  • Respond to any inquiry from external entities such as Department of Health, AIDS Institute, MCO, or other service providers on inquiries related to services.
  • Ensure that issues/concerns encountered or raised at/by the patients are resolved; to ensure smooth operation of the program, including troubleshoot any crisis with patients and/or care team.
  • Assign case coverage and/or provide coverage to the program staff as needed.
  • Work closely with other programs and departments in the organization to ensure a high-quality comprehensive continuum of services.
  • Actively participate in any organizational activities (team/multidisciplinary meetings, events, etc.).
  • Represent the agency at various venues to promote agency services.

Qualifications: 

  • BA/BS relevant field with 5 years full time experience in relevant field required. May consider advanced degree in social work, human services, mental health or equivalent with 3 years of full-time experience.
  • At least 2 years of supervisory or leadership towards paid staff experience.
  • Appreciation of and commitment to Apicha’s mission and values.
  • Experience working with target populations: individuals with low income, Serious Mental Illness, substance use disorders (SUDs), and/or chronic illness.
  • Experience with eCW, eSHARE, eICARE, Healthix, PSYKE preferred.
  • A desire and track record of doing all of the above in various environment – ownership, accountability, and humility.
  • Strong organizational skills with ownership mentality.
  • Self-motivated, outcome oriented, problem solver strongly preferred.
  • Must possess excellence verbal and written communication skills and ability to interact with all levels of external and internal customers with a high level of professionalism and culturally competent in representing Apicha CHC.
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).
  • Excellent customer service skills to assist in resolving patients and/or providers complaints.
  • Ability to adapt changing needs of communities and funder’s requirements.
  • Ability to work effectively in both a collaborative as well as individual setting.
  • Ability to travel to partner sites or community locations.
  • Occasional evening and/or weekend hours required.
  • This is an on-site position.
