Overview

Reporting to the Director of Violence Prevention, the Hospital based Intervention Program (HVIP) Case Manager plays a critical role in helping patients affected by violence move from crisis toward safety, stability, and long-term recovery. Working as part of One Brooklyn Health’s Hospital-Based Violence Intervention Program at the Brookdale campus, the Case Manager provides intensive, trauma-informed case management to patients and families during hospitalization and throughout their transition back into the community.

 

The position builds trusted relationships with individuals at a vulnerable and often life-changing moment, conducting comprehensive assessments, developing individualized service plans, coordinating hospital and community-based resources, and helping clients address barriers involving personal safety, housing, employment, education, behavioral health, healthcare access, and other social determinants of health.

 

As part of New York City’s Crisis Management System, the program uses a public-health approach to reduce reinjury, prevent retaliation, and address the underlying conditions that contribute to community violence. The Case Manager works closely with hospital staff, violence-prevention professionals, community partners, and service providers to support each client’s recovery and promote safer, healthier communities.


Responsibilities

  • Provide ongoing case management, including follow-ups, advocacy, and referrals to community-based resources.
  • Conduct comprehensive assessments to identify each client’s immediate and long-term needs, including personal safety, housing, employment, education, behavioral health, healthcare access, legal-service referrals, and family or social-support needs. Develop individualized, client-centered service plans in collaboration with clients, establishing appropriate goals, interventions, referrals, and follow-up activities. Coordinate services with physicians, nurses, social workers, care managers, behavioral health professionals, and other hospital personnel to support discharge planning and continuity of care.
  • Advocate for clients when navigating healthcare, social-service, educational, employment, and community-service systems, within the scope of the position. Connect clients with appropriate hospital, governmental, and community-based resources, including counseling, education, employment readiness, housing assistance, benefits, legal-support services, and other stabilization programs. Maintain complete, accurate, timely, and confidential case records in accordance with program and hospital requirements.
  • Track client progress, outcomes, and service utilization.
  • Participate in data collection needed for grant reporting, and program evaluation.
  • Provide crisis intervention, conflict de-escalation, and immediate supportive assistance in accordance with program protocols and professional boundaries.
  • Assist clients in developing individualized safety plans intended to reduce any risk of retaliation, possible reinjury, or further harm.
  • Develop and maintain effective working relationships with community-based organizations, violence-prevention partners, government agencies and other service providers.
  • Participate in community outreach activities, violence prevention initiatives, educational events, and other program-related activities as assigned.
  • Participate in interdisciplinary meetings, case conferences, supervision sessions, staff development activities, and program planning meetings.
  • Contribute to program development, workflow improvement, service evaluation, and quality improvement initiatives.
  • Perform other related duties consistent with the responsibilities and qualifications of the position.

Qualifications

Education:

  • Bachelor’s degree from an accredited college or university in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Public Health, Sociology, or a closely related field.
    OR
  • Associate degree from an accredited college or university in Human Services or a closely related field, combined with additional directly related professional experience.
  • Master of Social Work or another relevant graduate degree is preferred

Experience:

  • A minimum of two (2) years of professional experience providing case management, social services, victim services, violence-prevention services, community outreach, crisis intervention, or other closely related services, required.
  • Experience working with individuals affected by violence, trauma, justice-system involvement, housing instability, poverty, or other significant social and economic barriers is strongly preferred.
  • Experience working in a hospital-based violence intervention program, community violence intervention program, victim-services program, healthcare environment, or community-based organization is preferred.

Knowledge, Skills and Abilities:

  • Bilingual or multilingual proficiency in a language commonly spoken by the population served is preferred.
  • Strong verbal, written, interpersonal, advocacy, and organizational skills.
  • Proficiency with standard office technology, electronic documentation systems, and data-entry or case-management platforms.
  • Ability to prepare clear, accurate, timely, and confidential case documentation.
  • Ability to manage multiple clients, priorities, deadlines, and service-coordination activities in a fast-paced environment.
  • Ability to work collaboratively with multidisciplinary hospital teams, community organizations, governmental agencies, and external service providers.
  • Ability to maintain confidentiality and comply with applicable privacy requirements, hospital policies, and program procedures.
  • Ability to respond to hospital alerts and engage clients in inpatient, emergency-department, outpatient, and community-based settings.
  • Ability and willingness to travel within the community as required to perform assigned responsibilities.
  • Knowledge of trauma-informed, culturally responsive, client-centered, and strengths-based case-management practices.
  • Knowledge of community violence, social determinants of health, victimization, trauma, retaliation risk, and barriers affecting underserved communities.
  • Knowledge of community resources and referral networks related to housing, employment, education, behavioral health, healthcare, public benefits, legal-support services, and personal safety.
  • Ability to establish rapport and maintain effective working relationships with individuals who have experienced violence, trauma, crisis, or significant social and economic hardship.
  • Ability to conduct comprehensive assessments, develop service plans, coordinate referrals, monitor client progress, and maintain appropriate professional boundaries.
  • Demonstrated crisis-intervention, conflict-resolution, problem-solving, and de-escalation skills.
  • Ability to exercise sound judgment and respond appropriately to urgent, sensitive, or potentially volatile situations.

Licenses / Certifications:

  • Current New York State Licensed Master Social Worker credential is preferred
  • Certification or formal training in crisis intervention, conflict de-escalation, trauma-informed care, motivational interviewing, mental health first aid, or violence prevention is preferred.

Physical Requirements:

  • Ability to travel within the hospital facilities and community-based locations.
  • Ability to respond to hospital alerts and engage clients in various settings.
  • Position requires prolonged periods of sitting, standing, reaching, and walking throughout the working day. 
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
