BronxWorks is seeking one Case Manager to join a diverse and enthusiastic team at Park Haven, a supportive housing program located in South Bronx. Park Haven provides on-site case management services and permanent housing to 50 formerly homeless households, consisting of both single individuals and families with children. The ideal candidate will understand the Housing First model and will work comprehensively with their caseload to work on various tasks that include, but are not limited to, conducting home visits, escorts in the field to various appointments, and completing all necessary documentation in a timely manner. Qualifications (education, professional certifications or licenses, experience, proficiencies)Â
•Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment
•Bachelor’s degree in social work or another related field of studyÂ
•One year of case management or relevant work experience preferredÂ
•Engagement and detailed oriented skills are essentialÂ
•Proficiency in Microsoft Office suite and other standard business technology is requiredÂ
•Strong oral and written communication, time management and organizational skills are necessaryÂ
•Bilingual in Spanish is highly desirable
Essential Duties and ResponsibilitiesÂ
•Maintaining a caseload of 12-18 households (individuals and families) with chronic homelessness historyÂ
•Monitor and document client progress towards Treatment plan goalsÂ
•Conduct regular case conferences with clients and service providers as neededÂ
•Conduct monthly home visits for caseload and field work with clients when neededÂ
•Assist with managing and directing all walk-in clientsÂ
•Maintain familiarity with agency and city resources available to clientsÂ
•Document progress notes for all encounters and services provided to clientsÂ
•Participate in regular meetings, trainings and weekly supervision with supervisor and other support staffÂ
•Perform additional duties as assigned
Program Specific Responsibilities
•Completing ongoing Treatment Plans and Assessments for caseloadÂ
•Coordinate and connect clients to a variety of medical, mental health and behavioral health physicians and servicesÂ
•Assessing ongoing eligibility for various entitlements and resources for caseloadÂ
•Meeting monthly reporting requirements including timely documentation and engagement with full caseloadÂ
•Assisting clients with Representative Payee services including budgeting and financial educationÂ
•Assisting clients with Medication Monitoring on a daily, weekly, or as needed basisÂ
Skills/Abilities
 •Strong writing and oral communication skillsÂ
•Strong presentation skills and ability to represent the agency at functions and meetingsÂ
•Ability to collect and analyze client demographics and outcomeÂ
•Excellent organizational and teambuilding skillsÂ
•Ability to use a computer for prolonged periodsÂ

•Ability to stand, walk, or sit for long periods of timeÂ
•Ability to climb five flights of stairs, if required to conduct home visits and/or fieldworkÂ

•Ability to travel in the boroughs of New York City and its adjacent counties via public transportationÂ
•Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
