The typical US senior sees seven physicians across four practices plus insurers, pharmacies, family members, and facility staff. No one has the full picture. Fragmented data in incompatible systems leaves seniors and families managing chaos instead of focusing on living well.
Leland is building the first unified system of record for aging to help seniors live longer, healthier, and happier lives. With one complete record of a senior’s health and life, AI can intelligently analyze the full picture to surface risks, gaps, and opportunities, while the senior’s trusted human advocate guides the next best actions and removes friction so families can actually follow through. This turns fragmented information into proactive, coordinated support that extends healthy, independent years.
We have a multi-year funding runway and are launching pilots in 2026 with providers and already in active conversations with major US health systems. The company is led by a Stanford GSB founder with experience scaling new businesses at Sword Health and Homebase.
About the role
You will be Leland's first Patient Advocate: the one name and one number a family calls. You will carry our first families end to end and turn that work into the playbook every advocate after you is trained on.
Carry families end to end: Own the relationship. Know the whole story, make the calls, chase referrals, and close the loop after every discharge, referral, and medication change.
Work the queue and train the product: Accept, edit, or reject each AI-suggested next best action with judgment. Every decision teaches the intelligence layer what agents can safely do alone.
Run the AI leverage, verify everything: Use agents for booking, paperwork, and holds, but personally verify what reaches the family. Escalate anything requiring new clinical judgment to the physician.
Document to the billing standard: Capture actions, communications, and time so every month of service becomes a clean claim.
Write the advocate playbook: Turn the first months of live service into the scripts, checklists, and escalation rules the next advocates are hired against.
What we look for
Current, unrestricted licensure in good standing as an RN, NP, or PA.
3+ years of patient-facing work with older adults: navigation, care coordination, nursing, case management, or social work.
Based in New York City. This role is in person.
Fierce follow-through. Nothing you own gets dropped.
Spanish is a big plus.
Compensation
$85k to $105k base salary + equity and healthcare benefits. Monday to Friday, no shifts, no ratios.
