The Health System Command Center Pharmacy Coordinator supports centralized pharmacy operations through a virtual command center model that serves multiple hospitals and ambulatory care settings across the health system.

This role assists pharmacists and clinical care teams by performing medication reconciliation, patient outreach, medication access coordination, and prior authorization support while ensuring accurate medication management during care transitions.

The coordinator also supports Hospital-at-Home programs, facilitating medication coordination, patient communication, and medication access services to ensure safe medication use outside the hospital environment.

This position functions as part of a remote pharmacy services team designed to improve medication safety, patient outcomes, and care continuity across the health system.
