This position is responsible for coordinating the collection, maintenance, and release of medical records in compliance with federal, state, and organizational regulations, including HIPAA. The HIM Coordinator serves as a liaison between clinical teams, external providers, and regulatory agencies to support timely documentation submission and audit process. Key responsibilities include managing physician order tracking, facilitating the completion of death certificates, responding to medical records requests, preparing documentation for audits and appeals. The HIM Coordinator must demonstrate strong attention to detail, good communication skills, and a thorough understanding of healthcare documentation standards.

Responsibilities

  • Respond to requests for medical records from patients, healthcare providers, and authorized third parties.
  • Prepare complete medical records for extremal audits including CMS, NYS Department of Health, Managed Care organizations and patient appeals through Quality Improvement Organizations.
  • Collaborate with Health Information Management (HIM) departments at contracted inpatient facilities, including hospitals and skilled nursing facilities, to obtain copies of General Inpatient Level of Care documentation and upload them into the MJHS Hospice Electronic Medical Record (EMR) system.
  • Manage administrative component of the hospice physician orders including Certification of Terminal Illness, tracking outstanding orders and follow-up, obtaining signatures from the external physicians and ensuring compliance with organizational standards.
  • Coordinate and support the accurate and timely completion of death certificates for hospice patients in accordance with New York City and New York State Department of Health regulations.

Qualifications

  • High school diploma or equivalent; post-secondary education in health information Management or related field preferred.
  • Minimum one year’s experience in a healthcare setting or medical records department preferred.
  • Knowledge of MS Office (specifically MS Word and Excel).
