Job DetailsJob Location: Long Island City - Long Island City, NY 11101Position Type: Full-Time Non-ExemptSalary Range: $24.73 - $26.38 HourlyTitle: Intake Coordinator Unit: Admissions Reports to: Admissions Senior Director Status: Full Time; Regular; Non- Exempt Location: Long Island City Days/Hours: Monday to Friday, 9am to 5pm  Position Summary: The Intake Coordinator assesses each client’s eligibility for Fortune services by thoroughly screening each client, completing the agency’s comprehensive intake package, discussing Fortune services and the eligibility criteria for each, and making appropriate referrals to all Fortune programs and services. In addition, The Coordinator will need to gather resources in order to assist our clients with appropriate referrals.  Essential Duties and Responsibilities: Review new/returning client’s screening forms and make thorough assessments to determine whether the client is in need of Tier 1 (external) or Tier 2 (internal) programs and services; Conduct weekly audits and edit as required to ensure data fidelity; Assist Senior Intake Coordinator with Board review and data corrections; Assist in training new hires as Intake Coordinator; Perform crisis intervention as needed; Encourage and assist clients in planning, prioritizing, and achieving productive long-term and short-term goals; Complete full intake assessments for clients being referred to Tier 2 services; Enter and record all intake data into our database(s); Compose detailed individual Progress Notes highlighting all pertinent participant information for agency-wide use; Advocate on participants’ behalf and schedule internal and external referrals as needed; Communicate and interact positively with participants and staff at all times; Maintain neat, accurate, and timely documentation; Identify, research, and access external services and resources; Represent The Fortune Society at public speaking engagements; Perform other related duties as needed. QualificationsQualifications: A high school graduate or equivalent (some college preferred); Exceptional counseling and organizational skills; Solid verbal and written communication skills; Strong computer skills a must (Microsoft Office and Internet); Experience working with formerly incarcerated individuals; Bilingual in Spanish a plus.  We seek talented, dedicated individuals from all walks of life who possess a strong commitment to this mission.
