What You'll Do

As Medication Coordinator, you’ll be responsible for administering medication to clients as needed and ensuring that all prescriptions are documented and maintained in an orderly fashion. As a key member of the client service team, you’ll work cooperatively with other team members to carry out program goals and objectives and ensure the safety and wellbeing of clients on a daily basis.

Your responsibilities will include:

  • Working with clients and case manager in filling prescriptions and ensuring refills are ordered as necessary.

  • Ensuring physician orders are current, properly documented, and communicated to staff.

  • Referring clients to emergency care if needed, working with emergency responders and emergency room staff to coordinate emergency treatment, and coordinating treatment and discharge planning with hospital staff.

  • Reporting any verified or suspected abuse or neglect in accordance with policy and procedure.

  • Maintaining HIPPA protocols, confidentiality, and privacy regarding all client information.

  • Preparing clients for self-medication status and monitoring, as needed, health assessment referrals, etc.

  • Monitoring client medical equipment for safety and proper functioning.

You’re a great fit for this role if you have:

  • High School Diploma or equivalent OR equivalent experience and skills.

  • AMAP Certification.

  • CPR/First Aid Certification.

  • At least one year of experience in direct clinical work with psychiatrically disabled populations.

  • Preferred experience would include previous work with homeless population, work in residential settings and/or community health care, patient education/treatment adherence counseling, and group work.

  • Ability to work independently and as part of a team to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.

  • Bilingual (English/Spanish) preferred.

  • Computer literacy, particularly with Microsoft Office application.

Compensation

Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
