We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.


We are seeking a Case Manager/Mental Health Counselor for The Doe Fund's Crystal-Towers Residence. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households.

The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to residents funded by HASA. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.

 The Ideal Candidate:

  • You have a Master’s Degree in Social Work or related field, a license in Clinical Social Work and LMSW or LMHC required. 
  • You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness 
  • You have at least 5 years of experience in the mental health field
  • You have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues
  • You have solid computer skills – internet, Microsoft Office, and charting database 
  • You have great organizational, writing, and communication skills

 

Key Responsibilities:

  • Maintain a caseload and provide regular case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards 
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency 
  • Monitor and ensure residents  are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections 
  • Educate residents about approaches to mental health 
  • Attend ongoing training to enhance the services provided to the residents
  • Other duties as assigned
