The Outreach Specialist conducts home visits and outreach throughout Staten Island to identify new clients who are eligible for services. In addition, the Outreach Specialist re-engages with clients lost to follow up and works with community providers in identifying potential members. RESPONSIBILITIES: Coordinates with the Outreach Supervisor to identify clients to outreach. Conduct outreach services, including home visits, phone calls, and mailing letters with the goal to enroll all eligible clients. Complete progress notes and screening tools for clients. Driving agency vehicle regularly to provide outreach services. Complete documentation in the program's database. Qualifications Position requires High School Diploma or equivalent. Experience working with the Homeless/HIV population preferred. Bilingual Spanish/English a plus. Clean and valid driver's license with the ability to drive agency vehicle. Some weekends are required to attend community events. Must have exceptional communication skills both orally and in writing.
